Lots of nice numbers and tests, but the bottom line is that your assumption
that 720x480 in a DV frame represents a 4:3 image is wrong!
Of course the error is small, and it is probably more important to co-exist
with Premiere's foibles than to get it spot on, but we were talking
exactitudes!
For the hell of it I'll see what FCP does, because I suspect it is more
exact.

My miniDV camcorder FILLS the 720x480 frame. It doesn't contain 704x480
with 8 black pixel borders on each side.

As I totally expected, the 720x540 image is the CORRECT size to use with
DV in order to achieve a 4:3 final output on the TV screen.

It is nice to say that it is wrong without saying how/why it is wrong.
I'm not saying that 720x480 represents 4:3 in square pixels, but a 4:3
image in rectangular pixels.
